1/17/2024 EnchantaVaughn Approved QMHP-A inquired about Kailaâs thoughts, feelings, and emotions. QMHP-A recapped from the last session. QMHP-A utilized open discussion with Kaila to have her express concerns and anxieties. After discussion and review, QMHP-A taught Kaila basic concepts of how to identify and verbalize her emotional needs. QMHP-A assisted Kaila with contributing factors that triggers underlying feelings of depression. QMHP-A provided Kaila tentative examples of how her distorted thinking controls her day to day functions and how she thinks about situations, people, and things. QMHP-A reviewed journaling, and listening to music to help problem solving and conflicting behaviors. After discussion and review, QMHP-A challenged Kaila to face uncomfortable situations using coping skills to build confidence about herself. QMHP-A assisted Kaila with identifying the negative cognitive messages that feed her fear of rejection and failure by having her to list those triggers in order that causes her to experience symptoms of depression and other anxieties. Kaila appeared to be receptive to QMHP-Aâs interventions and services that were provided to her during the session. Kaila reported that she didn't realize that she was in her own way as it relates to her thoughts and feelings about people, things, and situations. She reported that she has been hurt for so long and she sometimes struggles with herself image as these are contributing factors to most of her depressive feelings. Kaila was helped to realize that her frequent remarks about herself impede on building positive thinking and building her confidence. Kaila admitted that she over thinks a lot to things by analyzing her own thoughts. Kaila discussed with QMHP-A that she was going to start walking and trying to also eat healthy so that she does not have extra factors that contribute to her depression.
